INSURANCE AND BENEFITS 

Please note, that we do not longer provide direct billing to your benefit company due to many changes to the plans. Recently it became very hard to follow up with the plans. But it does not mean that you are no longer eligible for massage to be covered by your benefit company. We will give you a receipt by the end of the treatment and you can submit it on your own and usually the frame time to get reimburse is 48 hours. But before you do so, please check your coverage and the percentage you are covered. In many companies you also may be required to bring a doctor referral for the massage.

CANCELLATION POLICY

If client needs to cancel or reschedule the appointment, it could be done with providing a minimum of 24 hours notice  If cancel less than 24 hours before scheduled appointment time, the client will be charged 50% of the appointment rate. If a client did not visit the appointment without notice, than will be billed at 100% of the appointment rate. Exceptions may be made depending on the circumstances of the cancellation.

PRIVACY POLICY

Like all medical professionals, I collect, use and disclose personal information in order to serve my clients.  The primary purpose for collecting personal information is to provide treatment. I collect information about a client’s health history, including their family history, physical condition, function and social situation in order to help me assess what their health needs are, to advise them of their options and then to provide the health care they choose to have.

I use clients’ personal health information primarily for the purpose of providing safe and effective massage therapy. Secondary purposes commonly include, but are not limited to:

  • disclosing treatment details to insurance companies for the purpose of verifying massage therapy benefit claims made by clients;
  • conferring with other regulated health care professionals regarding a client’s care; and
  • complying with requests from representatives of the College of Massage Therapists of Ontario, who may view client files as part of their quality assurance program.

I will disclose clients’ personal health information without their prior consent only when required to by law, or under circumstances granted by the Regulated Health Professions Act and PHIPA. In all other instances, I obtain clients’ written authorization prior to disclosing their health information. Clients are welcome to sign a form that provides ongoing consent to disclose treatment details to their insurers, for the purpose of verifying benefits claims.

Documents containing personal health information are kept securely in the appropriate patient records. Documents are securely disposed of when they are no longer in use, or have exceeded the legally-mandated retention period. Electronic files are secured on password-protected devices.
